Harmony's Continuum of Care Includes Memory Care for Residents with Dementia

Harmony Senior Services' Continuum of Care offers residents consistency with the same Executive Director, Dining, Programming, and compassionate care associates regardless of their current care level, whether it be Independent Living, Assisted Living, or Memory Care. 

Harmony's licensed healthcare professionals will conduct an assessment to create a personalized care plan for you or your loved one to ensure the quality of life and maximize independence. You can have peace of mind knowing that your loved one's needs are being met, including assistance with medications, help with scheduling healthcare appointments, and even a friendly escort to meals and activities. 

Medication Management

According to the CDC, about 350,000 patients need to be hospitalized each year due to adverse drug events. Seniors are especially prone to this because they are taking more medications. Harmony offers medication administration through licensed healthcare associates. Additionally, associates help residents to organize and manage refills and renewals and ensure that all medications are reviewed for adverse drug effects. 

Best Friends™ Approach

Harmony utilizes the Best Friends™ Approach in its Memory Care program. This approach is "grounded in the understanding that relationships are supremely important in dementia care and should reflect the essential elements of friendship, respect, empathy, support, trust, and humor," according to the Best Friends Health Profession Press. 

The Best Friends™ Approach promotes the dignity and individual rights of people with memory loss through knowing the history and preferences of each resident, communicating skillfully, and providing a supportive environment and meaningful engagement.

The nursing department has a daily meeting to review the residents' care, needs, and changes of conditions."
